Triana RiverHawks Skills Development Program Registration Agreement *

By registering my child for the Triana RiverHawks Skills Development Program, I, as the parent or legal guardian, acknowledge and agree to the following: I understand that participation in basketball training involves physical activity and carries inherent risks of injury. I voluntarily assume all risks related to my child’s participation and agree to release, discharge, and hold harmless the Triana RiverHawks program, its coaches, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising from injury, illness, or other damages that may occur during or as a result of participation. I certify that my child is physically fit to participate and has no known medical conditions that would prevent safe involvement in the program. In case of emergency, I authorize program staff to seek necessary medical treatment for my child and accept full financial responsibility for any such treatment. I also grant permission for the Triana RiverHawks program to photograph and/or record video of my child during activities and use these images and videos for promotional, educational, and social media purposes without compensation.
